O uso do cavalo com fins terapêuticos, segundo literatura médica européia, iniciou-se por volta de 1735. Em 1950, esses programas se estabeleceram na Inglaterra e logo depois nos outros países europeus. Na Alemanha e na Suíça a montaria vem sendo utilizada como forma de terapia há mais de 20 anos. O Deutsches Kuratorium für Therapeutisches Reiten, centro alemão especializado neste tipo de tratamento forma profissionais hipoterapeutas e edita regularmente a revista Terapeutisches Reiten. Por volta de 1960 foi iniciada a hipoterapia nos Estados Unidos. Em 1969, foi criada a Associação Americana de Hipoterapia para Deficientes, coordenando esforços para divulgar o método naquele país, que conta hoje com mais de 400 programas de hipoterapia.
