Drug utilization research in a municipality in the state of Espírito Santo: findings and implications related to the practice of polypharmacy

Abstract

Objective: To estimate polypharmacy prevalence, profile, and associated factors during the COVID-19 pandemic in Southern Espírito Santo residents. Methods: A cross-sectional study was carried out in the city of Alegre, between November and December 2021. A descriptive analysis was carried out for the sociodemographic and clinical characteristics of the interviewees, as well as the most used medications. The occurrence of minor polypharmacy was defined as the use of two to four medications and major polypharmacy as the use of five or more medications. Multinomial logistic regression models were used to analyze the association of sociodemographic and clinical variables with the occurrence of polypharmacy. Results: The most used drugs were those for the cardiovascular system, followed by the central nervous system and alimentary tract and metabolism, with the five most used drugs represented by dipyrone, losartan, hydrochlorothiazide, simvastatin and metformin. The prevalence of minor polypharmacy was
44.9% and of major polypharmacy, 20.2%. Factors associated with lower polypharmacy were: older age, female gender, self-declared white, worse quality of life, medical appointments in the last year, use of the Municipal Basic Pharmacy and presence of comorbidities. Factors associated
with major polypharmacy were the same as those associated with minor polypharmacy, with the exception of the variable gender and having only one disease. In addition, higher income, fair and poor/very poor self-perception of health, not self-medicating, problems with adherence to treatment and non-use of medicinal plants were factors associated with the occurrence of higher
polypharmacy. Conclusion: Several sociodemographic and clinical factors related to polypharmacy. Knowing them will aid health education measures to assist in promoting the rational use of medication and improve patients’ safety and experience while using them.
